Student Enrollment Application 2026
Complete and submit your enrollment application to reserve your student's spot at Smart Start Academy. Available at all campuses.
Health & Immunization Documents
As required by the NJ Office of Licensing, all students must submit current health and immunization records before their first day. Please have the following documents ready for your campus staff.
Required by NJ Office of Licensing
- Completed Immunization Record (NJ Schedule)
- Up-to-Date Physical Examination Form
- Lead Screening Results
- Health History Form
- Emergency Contact & Medical Authorization
All health documents must be current and signed by a licensed physician. Our campus staff will verify records upon submission.
State of NJ Health History Appraisal
The official New Jersey Health History Appraisal form required by the NJ Office of Licensing. Must be completed and signed by your child's licensed physician prior to enrollment.
Universal Health Form
The Universal Health Form required for all enrolling students. Complete with your child's physician and submit to your campus prior to the first day of attendance.
